Caremark administers the Prescription Drug program on behalf of Mid-West National Life Insurance Company of Tennessee and has contracted with over 55,000 participating pharmacies from coast to coast. This is prescription coverage made easy - there are no claim forms to fill out and submit; no waiting for reimbursement when using participating pharmacies. Benefits are received when you present your ID card at the time of purchase at participating pharmacies. For non-participating pharmacies, a claim form will need to be submitted prior to receiving reimbursement.
