Basic Medical/Surgical Expense Plans
designed to be affordable for those currently without health coverage because your health is too important for you to go without insurance. If you're currently uninsured or looking for a better fit in an insurance plan, these plans could be the solution you've been waiting for. You can customize our basic plan with optional add-ons that work best for you and your family. These plans are available without pre-authorization requirements or network restrictions; you'll be able to choose the doctor and hospital of your choice.
PPO Plans1
- offering you catastrophic health insurance coverage using your choice of providers. Mid-West has partnered with some of the largest provider networks nationwide plus numerous local networks so that you can choose from many providers across the country. When you use a PPO provider you can experience greater benefit - for instance, we've negotiated provider arrangements that may let you reduce your out-of-pocket expenses. If you choose a non-PPO provider, however, benefits will be paid at the out-of-network benefit levels. A list of participating PPO providers is available to you at any time through the web site and by contacting your agent or the company's customer service team. There is no primary care physician or pre-admission authorization required.
1Your benefits may be reduced on PPO plans if you use an out-of-network provider. Optional benefits require additional premium and availability varies by state. In Louisiana this is a Limited Hospital Expense plan.
